New Student – Parent/Guardian Process Checklist

  1. Parent/Guardian visits the New Student Enrollment section of the Fair Grove School website (this page)
  2. Parent/Guardian completes the online NEW STUDENT PROOF OF RESIDENCY FORM

    The school will review and process the information in the proof of residency form

  3. Parent/Guardian will receive a Lumen email with a temporary invitation code to log into Lumen to complete two online forms for new students:
    1)Demographics Form
    2)Permission to Release Student Information (Kindergarten will not do this section)

    The school will process the information in the two forms listed above and will generate a Lumen parent portal and send credentials email to parent

  4. IMPORTANT Parent/Guardian must log in and complete ALL the registration forms in the regular parent portal

    Directions for filling out the additional Lumen registration documents


    Visit the Back-to-School Page on the website for important information, and download the Fair Grove R-X School District App to your phone from your app store and set the notification preferences on the app to receive updates throughout the school year.
