Jordan Valley's Mobile Dental Unit is coming to the Fair Grove Middle School & High School next Monday, March 26th. Below is a link to the form that parents will need to download, print and send with their student(s) after they fill it out and have it back to the school by this Friday, March 23rd. If you do not have access to a computer and need a paper copy please pick one up from the building secretary or the nurse.
